Documents needed to obtain a stamp in your passport (known as formal requirements):

• application in 1 copy
• 4 photographs,
• photocopy of a valid travel document (original for inspection). In a particularly justified case, if you do not have a valid travel document and do not have the ability to obtain one, you can provide another document to prove your identity,
• attachment No. 1 to the application completely filled out by the employer, signed by a person authorized to represent the company.

You will also need confirmation that you have paid the stamp duty – PLN 440 – to start the proceedings.
